5th Grade Yearbook Ads

Are you ready to purchase a yearbook ad to celebrate your 5th grader? It’s easy! Follow these two simple steps to pay for your ad and provide details about your 5th grader.

1. Complete the form below to place your order and provide payment for your ad.
2. After placing your order, click the link on the confirmation page to upload your student’s photo, name, and a special message for their yearbook ad. Instructions are provided on the confirmation page displayed after clicking the “Place Order” button below.

All orders must be placed and student information provided by April 4, 2025. Please be sure to place your order and submit your details carefully. All sales are final and the PTO is not responsible for errors or mistakes in your ad content.

IMPORTANT NOTE: Once you complete your order, you will be taken to a confirmation page that includes a link to a Google form where you can upload your student’s photo and provide your student’s name and a special message. This is the content that will appear in your yearbook ad. We cannot create your ad without this content. Please submit payment and ad content no later than April 4th, 2025.
